Instructions
Who can use this form?
- UIUC employees: academic professionals, civil service, extra help staff, faculty, other staff
- Student employees: postdoctoral scholars, GAs, TAs. If you are a Fellow seeking an accommodation, please contact DRES.
- Applicants for employment at the university.
- University Housing residents who are requesting an emotional support animal and are not students.
- Visitors and guests attending events on UIUC grounds (when directed to use this form).
Who should not use this form?
- If you previously filed an accommodation request with our office (since 2020), please log in using your NetID and Password to submit your request.
- Students who are seeking classroom- or housing-related accommodations should contact DRES.
- UI System Office employees must use the System Reasonable Accommodation Form.

- This form must be completed in a single setting. It cannot be saved and returned to later.
- You may submit medical documentation with your form. Forms for supplying medical documentation can be found here. Your medical information will not be shared outside of the Division.

You may submit up to two accommodation requests with this form.
Disability, Religious, and Pregnancy-related Accommodations:
- Select one of these accommodation types to request a workplace accommodation related to a disability, religious conviction, or pregnancy-related status. Medical documentation is not required for religious accommodations and for most pregnancy accommodations.
Parking Accommodations:
- If you are seeking a parking accommodation due to pregnancy or a disability, please select "Parking" from the drop-down menu.
Travel Accommodations:
- If you are seeking an accommodation related to travel for university business, please select ‘Travel’ from the dropdown menu.
University Housing Residents or Applicants:
- Use this form if you are a non-student living in, or applying to live in, University Housing, and you are requesting an accommodation for an emotional support animal (ESA). Students who live in University Housing, please contact University Housing. All other students, contact DRES.

Accommodations are not retroactive. If you have already traveled and would like to be reimbursed for an additional expense you paid as the result of a disability or pregnancy, please speak with your department. We cannot guarantee that a reimbursement will be granted; departments are not obligated to provide reimbursements when there is no reasonable accommodation in place. The Accessibility and Accommodations Division is not able to authorize retroactive reimbursements.
AAD is currently experiencing a high case load; if possible, please complete this form at least two months before your expected travel date.
(9-digit ID) - Employees MUST provide their UIN in order for the Division to verify employment status. If you do not know your UIN, please contact your HR representative. If you are a member of the public and do not have a UIN, please enter "N/A."
